McLaren Health Care seeks an RN Care Manager in Quality Management / Behavioral Health. You will assess, plan, implement, coordinate, monitor, and evaluate options to meet patients’ health needs, using clinical and community resources to promote quality, cost-effective outcomes and reduce hospitalizations.

You will conduct telephonic and in-person assessments with an interdisciplinary team, target high-risk/geriatric populations, and support self-management goals within population health

Competenze

• RN with an unrestricted Michigan license.
• Three years of clinical nursing experience serving chronically ill patients.
• Understanding of Population Health Management and managed care initiatives.
• Ability to support patient self-management goals in a team setting.
• Culturally competent care and strong problem-solving skills.
