Job summary

Providence Redwood Memorial Hospital in Fortuna, CA is seeking a Care Manager RN on a Per Diem basis for 8-hour Day shifts. This role focuses on coordinating care for complex patients across the continuum, communicating with staff and external resources to ensure appropriate utilization of resources.

The ideal candidate holds an Associate's Degree in Nursing, CA RN license, at least 2 years acute care experience, and 1 year in care management or related orientation programs.

Qualifications

• Associate's Degree in Nursing or nursing diploma.
• California RN license on hire.
• 2 years of acute care in Medical-Surgical or Inpatient setting.
• 1 year in care management or utilization review or completion of TIPS/Case Management Orientation.

Responsibilities

• Carry out effective case management for identified patients across the continuum of care.
• Coordinate and communicate with medical staff and resources to ensure appropriate resource utilization.
• Plan, negotiate, procure and manage care for complex patients and families.
