Formación

• Associate's Degree in Nursing or a nursing diploma upon hire.
• California Registered Nurse License upon hire.
• 2 years of acute care experience in Medical Surgical or Inpatient setting.
• 1 year of care management or utilization review experience, or completion of TIPS/Case Management Orientation.

Responsabilidades

• Carry out effective case management functions for identified patients across the continuum of care.
• Communicate daily with medical staff, hospital personnel and outside resources to ensure appropriate utilization of resources.
