Job summary

Northwell Health is seeking a registered nurse to execute an age-specific care plan using the nursing process, collaborate with physicians, and direct care team activities on the unit. You will assess physiologic and psychosocial health, set outcomes, plan and implement care, monitor progress, and provide discharge planning and education to patients and families.

The role requires a NY or CT RN license and BLS; nursing expertise across specialties is valued.

Qualifications

• Graduate from an accredited School of Nursing.
• Associate’s Degree in Nursing, required.
• Bachelor’s Degree in Nursing, preferred.
• Minimum 1-2 years of professional nursing experience in the related specialty.
• Current license to practice as a Registered Professional Nurse in New York State and/or Connecticut, required.
• BLS required, plus specialized certifications as needed.

Responsibilities

• Assesses the patient’s physiologic health status.
• Assesses the patient’s psychosocial health status and coping needs.
• Identifies patient outcomes and goals.
• Formulates the plan of care and priorities.
• Implements the plan and administers treatments safely.
• Evaluates care and outcomes for improvement.
• Collaborates with physicians and care team.
• Performs grade I–IV decubitus care.
• Participates in patient and family education.
• Performs related duties as required.
• Maintains documentation and communication per policy.
