Job summary

Qualifications

• Unrestricted NC RN license is required.
• Must be a resident of NC.
• 4+ years in pediatric/maternity care or 2+ years in community-based pediatric/maternity care.
• Must be able to travel within the assigned region to visit Medicaid members in homes or other settings.
• Intermediate skills in Microsoft Office Suite.

Responsibilities

• Assess, plan and implement care management interventions tailored to each member.
• Identify and initiate referrals for healthcare and community services, including financial and psychosocial supports.
• Develop and implement care plans as a single point of contact across the continuum of care.
• Communicate required health information to stakeholders to ensure coordinated care.
• Advocate for members and families to align with their needs and choices.
• Provide field-based visits to members locally (up to 25%), up to 120 miles round trip.
