Job summary

UnitedHealthcare, a member of UnitedHealth Group, seeks a Care Manager in North Carolina to coordinate care for members locally. You will advocate for the best services and may conduct field visits up to 25% with mileage reimbursement.

Requirements include an unrestricted NC RN license, NC residency, and 4+ years in pediatric/maternity care (or 2+ years in community-based care management). Travel within the region, MS Office proficiency, and a valid driver’s license are needed.

Qualifications

• Unrestricted RN license in the state of North Carolina.
• Must be a resident of NC.
• 4+ years of experience in pediatric/maternity care or 2+ years in community-based care management.
• Ability to travel within the assigned region to visit Medicaid members in homes or other settings.
• Intermediate skills in Microsoft Office Suite.
• Willing to work with pediatric and maternity populations.
• Driver's license and reliable transportation.

Responsibilities

• Assess, plan and implement care management interventions tailored to each member's needs.
• Identify and refer for healthcare and community-based services (financial, psychosocial, etc.).
• Develop and implement care plan interventions across the continuum of care as a single point of contact.
• Communicate health-related information to stakeholders for coordinated care.
• Advocate for members and families to ensure their needs are represented.
• Conduct field-based visits locally up to 25%, with mileage reimbursement.
