Job summary

Qualifications

• Unrestricted RN license in NC.
• Resident of NC.
• 4+ years in pediatric/maternity care or 2+ years in community-based care management for pediatric/maternity.
• Ability to travel within assigned region to visit Medicaid members.

Responsibilities

• Assess, plan and implement care management interventions tailored to each member.
• Identify and initiate referrals for healthcare and community-based services.
• Develop and implement care plans throughout the continuum of care.
• Communicate with stakeholders to ensure coordinated care.
• Advocate for members and families to ensure needs are met by the care team.
• Provide field-based visits up to 25%, up to 120 miles round trip.
