Responsibilities

• Complete comprehensive member assessments within regulatory timelines, including home visits.
• Facilitate waiver enrollment/disenrollment processes and develop care plans with care team.
• Monitor and adjust care plans; document interventions and outcomes.
• Promote integration of LTSS, behavioral health, and community resources.
• Assess medical necessity and authorize waiver services.
• Coordinate ICT meetings for service approval/denial.
• Provide education and motivation during member contacts.
• Identify barriers to care and assist with psycho/social and medical obstacles.
• Identify incidents and develop prevention plans; share insights with peers.
• 25-40% local travel may be required.
