Job summary

Molina Healthcare is seeking a skilled Registered Nurse to serve as a care manager, coordinating long-term services and supports (LTSS) for high-need members. You will collaborate with a multidisciplinary team to assess needs, develop and monitor care plans, and promote integrated services across behavioral health and LTSS.

Some local travel is required. The role requires an active RN license, 2+ years in health care with care management experience, and strong communication skills.

Qualifications

• Active RN license in state of practice.
• 2+ years in health care with care management experience or LTSS.
• Experience with persons with disabilities, chronic conditions, or substance use disorders is a plus.

Responsibilities

• Complete comprehensive member assessments within regulatory timelines, including in-person visits as required.
• Facilitate waiver enrollment/disenrollment and develop care plans with the member and team.
• Monitor care plan effectiveness and document interventions and goals.
