Qualifications

• Preferred: Case Management Certification.
• Proficient in basic computer tools (Word/Excel).
• Ability to manage care transitions and discharge planning.

Responsibilities

• Identifies patients using screening criteria and conducts comprehensive evaluations.
• Develops, adjusts, and coordinates care plans addressing medical and social needs.
• Guides self-management using motivational interviewing and coaching.
• Provides education to healthcare teams and patients; advocates for patient rights.
• Collaborates with multidisciplinary teams to ensure timely, high-quality care.
• Leads initiatives to improve care transitions and policy adherence.
• Provides Goals of Care education for informed decision-making.
• Tracks metrics, analyzes trends, and ensures regulatory compliance.
