Job summary

Heritage Health Network is seeking a Registered Nurse Care Manager (RNCM) to provide clinical oversight and guidance within our Enhanced Care Management (ECM) program. The RNCM will review member charts, assess care plans, and provide clinical guidance to a team of Lead Care Managers, combining remote clinical work with field-based activities as needed.

The role requires California RN licensure, 2 years of nursing experience, and at least 1 year in care management or leadership, with strong

Qualifications

• Active RN license in California.
• Minimum 2 years of nursing experience.
• At least 1 year in care management or leadership.
• Experience with vulnerable populations and complex medical/social needs.
• Strong clinical assessment and care planning skills.
• Proficiency with EMR/EHR systems.

Responsibilities

• Provide clinical oversight and guidance to Lead Care Managers.
• Review member charts and assess care plans to identify gaps.
• Coordinate with multidisciplinary team for ECM care.
• Conduct field-based visits and in-person member engagement as needed.
• Ensure documentation aligns with Medi-Cal ECM guidelines and regulations.
