Job summary

Heritage Health Network seeks a Registered Nurse Care Manager (RNCM) in the Palm Springs area. The role oversees care plans, reviews member charts in EMR/EHR, and coordinates with care teams to improve outcomes.

Hybrid work flexibility is offered, with field visits and in-person engagement as needed. Qualified candidates hold an active CA RN license, at least 2 years of nursing experience including care management, and strong clinical judgment for vulnerable populations.

Qualifications

• Active RN license in California.
• Minimum 2 years of nursing experience.
• At least 1 year in care management or leadership.
• Experience with vulnerable populations and complex needs.
• Proficient with EMR/EHR systems.
• Excellent communication and collaboration skills.
• Willingness to perform field-based work.

Responsibilities

• Clinical Oversight & Quality Assurance: guide care plans and assess medical history and adherence.
• Care Coordination & Field Support: partner with multidisciplinary teams and conduct field visits.
• Regulatory Compliance & Documentation: ensure alignment with Medi-Cal ECM guidelines and proper recordkeeping.
