Job summary

Responsibilities

• Leads the coordination of patient care with other disciplines within the care team.

• Ensures the interdisciplinary care plan aligns with the patient’s clinical course and discharge needs.
• Discusses resource utilization, treatment plans, and length of stay with physicians.
• Coordinates transfer of patients to tertiary centers as needed.
• Provides oversight for regulatory and accreditation data collection and compliance.
