Job summary

Qualifications

• RN licensure in the state of practice is required
• Case management certification is preferred
• Experience in coordinating patient care with multidisciplinary teams is preferred

Responsibilities

• Leads coordination of patient care with the care team, monitoring appropriateness and timeliness of care
• Ensures the interdisciplinary care plan aligns with the patient’s clinical course and discharge needs
• Discusses with physicians resource utilization, treatment plans, LOS estimates and discharge plans for complex patients
• Coordinates transfer of patients to tertiary centers and transfer of information for continuity of treatment
• Provides oversight for regulatory and accreditation data collection and manages ED utilization focus reviews
