Job summary

Entyre Care is seeking a detail-oriented Care Manager to conduct home visits, train and support caregivers, and ensure high-quality, compliant home care for clients and their families. You will monitor health, develop personalized care plans, and coordinate with physicians and community resources.

Qualifications

• RN license is required.
• At least 1 year of in-home healthcare experience with case management and care planning.
• Willingness to travel within the assigned geographic area; valid license and auto insurance.
• Experience supervising caregivers is preferred.
• Proficiency with MS Office and case management systems.

Responsibilities

• Home visits to assess needs and craft personal care plans.
• Train and support caregivers to deliver quality home care.
• Monitor client health and report changes to physicians and team.
• Coordinate services with primary care, resources, and community partners.
• Ensure ongoing, compliant care and smooth transitions between services.
• Engage in community events to raise awareness and recruit caregivers.
• Model Entyre Care values and uphold mission and standards.
