Job summary

Entyre Care is seeking a licensed Registered Nurse (RN) to join our Virginia home-care team, conducting in-home assessments and care planning for elders and adults with disabilities. This full-time hybrid role combines field visits with office collaboration to deliver high-quality, client-centered care.

You will travel regularly within the state, complete Medicaid/DMAS documentation, and coordinate with PCPs, family caregivers, and case managers to ensure safe, effective transitions and ongoing

Qualifications

• Active Registered Nurse license in Virginia.
• Minimum two years of direct elder/adult caregiving experience.
• Ability to travel within Virginia for home visits (valid license and auto insurance).
• Conduct comprehensive in-home nursing assessments and document findings.

Responsibilities

• Conduct home visits to assess and create personalized care plans.
• Train and support caregivers to meet clients' needs.
• Monitor clients’ health and coordinate with PCPs.
• Facilitate smooth transitions and ongoing care for clients.
• Maintain compliance with Virginia HCBS and Entyre Care protocols.
• Complete timely clinical documentation in the EHR.
