Qualifications

• Active RN license in the state of employment.
• Minimum two years of direct care experience with elders or adults with disabilities.
• Ability to travel within assigned geographic area; valid driver’s license and auto insurance.
• Experience in case management, care planning and assessment in home care settings.
• Proficiency with MS Office and web-based applications; familiarity with case management systems.

Responsibilities

• Conduct home visits to assess and create personalized care plans.
• Train and support caregivers to meet clients’ needs.
• Monitor and report clients’ health and coordinate with primary care physicians.
• Facilitate smooth transitions and ongoing care for clients.
• Maintain compliance with regulations and standards.
• Full-time hybrid position leveraging nursing assessment, care planning and case management skills.
