Job summary

Responsibilities include managing care plans, coordinating transitions, direct patient care, vaccinations, and documenting progress in the EHR. Evening, weekend, and holiday shifts may be required.

Qualifications

• RN license valid in Massachusetts.
• Bachelor’s degree in Nursing from an accredited college.
• 1–2 years of clinical experience in healthcare.
• Current CPR certification.
• COVID-19 booster and yearly flu vaccine required.

Responsibilities

• Coordinate and manage care for MAT program patients with PCPs.
• Develop and update care plans to improve outcomes and reduce ER visits.
• Maintain communication with the care team about patient status.
• Provide direct patient care including assessments and vaccinations.
• Document patient care and progress in the EHR; may require evenings/weekends/holidays.
