Job summary

CareSource/Commonwealth Care Alliance (CCA) is seeking an integrated care clinical manager in Massachusetts to provide monitoring, follow-up and care management to dually-eligible enrollees with complex needs. The RN will coordinate health services and community resources to improve outcomes and quality of life.

The role requires leading the ICT, collaborating with clinicians and community partners, and traveling to member homes and sites to ensure seamless care delivery.

Qualifications

• Associates of Science degree in nursing from an accredited program.
• Current unrestricted RN license in Massachusetts.
• Experience in care coordination or case management for dual-eligible populations preferred.

Responsibilities

• Engage enrollees in home and community settings to establish complex care management relationships.
• Lead interdisciplinary care team (ICT) to create holistic care plans.
• Coordinate with providers and community resources to ensure seamless care transitions.
• Follow up after hospitalizations to prevent readmissions.
• Travel to member, provider and community visits as required.
