Job summary

UNC Health Care's Rex Case Management Services in Raleigh, NC seeks a registered nurse to provide ongoing assessment, planning, implementation and evaluation of patient needs to promote continuity of care and cost effectiveness. Qualifications

• Two years of health care experience as a Registered Nurse.
• Strong assessment and critical thinking skills.
• Licensed RN in North Carolina.

Responsibilities

• Identify cases and prioritize daily patient care per guidelines.
• Attend and participate in CAPP meetings for discharge planning.
• Attend weekly Complex Care Meetings and present patient data.
• Consult with multidisciplinary team to resolve discharge barriers.
• Coordinate family meetings and educate on community resources.
• Monitor patient progression and readiness for discharge.
• Document activities accurately in the EHR.
• Maintain confidentiality and comply with policies.
• Support performance improvement initiatives and training.
