Job summary

Providence St. Joseph Medical Center in Burbank, CA is seeking a Case Manager RN on a per diem basis for 8-hour day shifts.

This role collaborates with patients, families, and a multidisciplinary team to coordinate care and transitions across settings. The ideal candidate holds a California RN license, has 2+ years in healthcare and 2+ years in Case Management or the TIP program, and may have a Master's degree or CCM/ACM credentials.

Qualifications

• Bachelor's Degree in Nursing or equivalent education/experience.
• California RN License upon hire.
• Minimum 2 years experience in healthcare field.
• Minimum 2 years experience in Case Management or TIP program.

Responsibilities

• Coordinate care among patients, caregivers, nurses, social workers, physicians and payers.
• Facilitate transitions in care and care coordination across settings.
• Support patient self-determination and address social determinants of health.
• Document care plans and maintain accurate case management records.
