Qualifications

• 2 years experience in healthcare field (acute, ambulatory, post-acute, etc.).
• 2 years experience in Case Management or TIP program experience.

Responsibilities

• Coordinate care across patients, caregivers, nurses, social workers, physicians, payers, and support staff.
• Facilitate effective transitional care management and timely communication across care settings.
• Ensure compliance with licensure, facility policies, and care management standards.
