Job summary

Northwest Medical Center in Tucson, AZ is seeking a Care Manager - RN to coordinate discharge planning, transitions of care, and case management activities to ensure optimal patient outcomes. You will work with interdisciplinary teams, review medical records for appropriateness and medical necessity, and ensure compliance with federal, state, and accreditation standards.

The role requires RN licensure, a Bachelor’s degree preferred, and 2–4 years of clinical nursing and care management

Qualifications

• Knowledge of case management principles, discharge planning, and transitions of care.
• Familiarity with federal, state, and Joint Commission standards for case management.
• Excellent communication and interpersonal skills with patients, families, and teams.
• Ability to assess complex situations, identify solutions, and implement care plans.

Responsibilities

• Conduct daily reviews of medical records to assess admission appropriateness and utilization.
• Collaborate with interdisciplinary teams to coordinate patient care and transitions.
• Develop and implement discharge plans, coordinating post-hospital placement and social services.
• Refer cases to physicians or managers when criteria are not met and ensure timely interventions.
• Serve as liaison with community agencies to facilitate seamless transitions for discharged patients.
• Lead interdisciplinary meetings to address patient care needs and plan care accordingly.
• Maintain accurate documentation of case management activities and compliance with reporting requirements.
