• Knowledge of federal/state standards related to case management.
• Excellent communication and interpersonal skills.

Responsibilities

• Conducts daily reviews of medical records to assess admission and utilization.
• Collaborates with interdisciplinary teams to coordinate patient care and transitions.
• Develops and implements discharge plans and coordinates post-hospital services.
• Refers cases to physicians/managers when criteria are not met.
• Serves as liaison with community agencies to facilitate transitions.
• Facilitates interdisciplinary meetings to address patient care needs.
• Maintains accurate documentation of case management activities.
• Identifies and refers cases to protective services as needed.
• Provides information to patients/families about discharge planning.
