Job summary

Physicians Regional Medical Center - Collier campus is seeking a Registered Nurse (RN) to join our Care Manager ER team. The role partners with the emergency department to assess admission needs and manage transitions of care, with a focus on efficient patient flow and appropriate disposition.

Qualifications include a BSN preferred and 1–3 years of acute/hospital care plus experience in care management or utilization review.

Qualifications

• BSN preferred.
• 1-3 years of experience in an acute care or hospital setting.
• 1-2 years of care management or utilization review preferred.

Responsibilities

• Assess patients for medical necessity and placement using criteria.
• Identify patients for alternative settings to avoid admissions.
• Guide transitions of care, including hospice/post-acute placement; obtain documentation.
• Verify next of kin and emergency contacts; document accurately.
• Collaborate with ED clinicians, hospitalists, care managers, and social services for seamless transitions.
• Perform other duties as assigned.
• Maintain regular attendance.
• Comply with policies and standards.
