Qualifications

• Nursing degree with active RN licensure.
• 2-4 years of clinical nursing experience in hospital/home health/nursing facility.
• 2-4 years of care management experience preferred.

Responsibilities

• Coordinate discharge planning and transitions of care.
• Review medical records for appropriateness and necessity.
• Collaborate with interdisciplinary teams to coordinate patient care.
• Develop and implement discharge plans and post-hospital services.
• Maintain accurate documentation of case management activities.
• Refer cases to physicians or managers when criteria are not met.
• Identify and refer cases to Protective Services when needed.
