Amedisys in Oregon is seeking a Nurse Case Manager to guide patients and families through the care continuum, coordinating care, supporting discharge and transition planning, and managing ongoing outpatient care under supervision of an RN or MD.

You will perform assessments, coordinate referrals, teach self-care, supervise LPNs and HHAs, and participate in ongoing clinical development. The role emphasizes high-quality, patient-centered home health services in a supported team environment.

Formación

• Current RN license, valid in the state(s) you work in.
• Valid driver's license, reliable transportation and liability insurance.
• Current CPR certification.
• 1+ year of clinical experience as a Registered Nurse (RN).

Responsabilidades

• Performs patient assessments and collaborates with the care team to develop and implement a plan of care
• Makes referrals to other disciplines as indicated by the patient's needs or documents rationale for not doing so
• Promotes patient health and independence through teaching and rehabilitative measures; assists patients in self-care
• Supervises LPNs and HHAs
• Completes documentation timely and accurately
• Communicates patient progress to the clinical manager and care team
• Plans and provides staff education
• Performs on-call responsibilities and on-call services to patients/families
• Participates in clinical development and continuing education programs
• Other duties as assigned
