Job summary

Advocate Health is seeking a proactive RN Case Manager for an inpatient Summit site, 0.6 FTE, on-site Monday–Friday with a weekend rotation and holidays off. You’ll assess, plan, coordinate discharges, and support utilization management for admitted patients.

Licensed RN in the state, BSN degree, and at least 2 years of clinical nursing experience are required. Expect collaboration with social work, physicians, and the care team to optimize care transitions.

Qualifications

• Registered Nurse license issued by the state where practicing.
• Bachelor's Degree in Nursing.
• Minimum 2 years of clinical nursing experience.

Responsibilities

• Conduct comprehensive assessments and develop timely care plans and interventions.
• Collaborate with patients, families, and the multidisciplinary team to meet goals.
