Advocate Health is seeking a proactive Registered Nurse (RN) to serve as an inpatient RN Case Manager at the Summit site, 0.6 FTE. The ideal candidate holds a RN license in the state, a Bachelor’s Degree in Nursing, and at least 2 years of clinical nursing experience.
You will partner with patients, families, and the care team to assess needs, coordinate services, plan discharges, and support utilization management while ensuring regulatory compliance and high-quality care.
Seeking a proactive, self-directed individual with excellent organizational skills and the ability to work both independently and collaboratively. Strong interpersonal communication, teamwork, and adaptability are essential for success in this role.
Help patients transition safely through their hospital stay. Partner with patients, families, and the care team to assess needs, coordinate services, plan discharges, and support utilization management.
License/Registration/Certification: Registered Nurse License issued by the state in which the Team Member practices.
Level of Education: Bachelor’s Degree in Nursing
Years of Experience: 2 years of clinical nursing experience
