Job summary

NLC Support is seeking a Registered Nurse in New Jersey to create and manage plans of care for individual patients, overseeing Licensed Practical Nurses and delivering patient-centered care in a group home setting.

You will supervise care, coordinate with medical providers, assist with medications, maintain records, and participate in interdisciplinary team meetings. Flexible work-from-home options are available and NJ RN license with CPR certification is required.

Qualifications

• Licensed RN in NJ with current credentials.
• First aid/CPR certification.
• Strong interpersonal and client-care abilities.
• Flexible work-from-home options available.

Responsibilities

• Create care plans for individuals and oversee LPNs.
• Oversee medication administration and education.
• Monitor and document patient care and follow-ups.
• Coordinate with outside medical providers to ensure continuity of care.
• Maintain accurate medical/dental/other records for individuals served.
• Intervene in emergencies and attend IDT meetings.
• Support independence and normalization for individuals served.
• Provide training and support to staff on medical topics.
• Act as liaison to health professionals to coordinate care.
