Independent Living Systems, llc in Clearwater, FL is hiring an RN Care Manager to coordinate care for Medicaid and Medicare Special Needs Program members. You will assess needs, develop person-centered plans, and connect members with services to promote independence and quality of life.

In this full-time on-site role, you will collaborate with multidisciplinary teams, monitor progress, and advocate for members within clinical guidelines and program requirements to support optimal outcomes across

Competenze

Florida-licensed RN with at least 2 years serving disabled and elderly populations.
Minimum 2 years in care management or care coordination.
Proficient with electronic health records and basic computer applications.

Coordinate access to services and ensure continuity of care across settings.
Monitor progress and adjust care plans as needs change.
