Independent Living Systems, LLC is seeking an RN Care Manager in St. Petersburg, FL, to coordinate care for Medicaid and Medicare Special Needs Programs.

The role focuses on person-centered planning, collaboration with multidisciplinary teams, and advocacy to ensure timely access to services and optimal outcomes. The RN Care Manager will assess needs, develop care plans, coordinate services, and monitor progress while maintaining compliance with clinical guidelines and program requirements.

Competenze

• State of Florida licensed registered nurse with at least two years of experience with disabled and elderly populations.
• At least 2 years in care management or care coordination.
• Proficiency in electronic health records (EHR) and basic computer applications.
