Independent Living Systems (ILS) is seeking an RN Care Manager in Pinellas Park, FL to coordinate care for Medicaid and Medicare Special Needs Programs. You will assess needs, develop person-centered plans, and partner with providers and families to ensure seamless service delivery.

The role emphasizes advocacy, care coordination, and adherence to program guidelines to promote health and quality of life for vulnerable populations.

Competenze

• Florida RN license with 2+ years of experience with disabled and elderly populations.
• 2 years’ experience in care management or related healthcare role.
• Proficiency in electronic health records (EHR) and basic computer applications.
• Preferred CCM certification or equivalent.

Mansioni

• Assess member needs and develop person-centered care plans.
• Coordinate services and supports across the care continuum.
• Monitor progress and adjust care plans as needed for ongoing wellness.
• Collaborate with healthcare providers, social workers and families.
• Educate members on available resources and disease management.
• Maintain compliant documentation and quality standards.
• Stay informed on current treatment trends and community resources.
