Responsibilities

• Assessment of patients using screening criteria and comprehensive evaluations.

• Education & Advocacy: educate teams and patients, advocate for patient rights, support self-care.
• Collaboration: work with multidisciplinary teams to ensure timely, high-quality care.

• Advanced care planning: provide Goals of Care education for informed decision-making.
• Data analysis & compliance: track metrics, analyze trends, ensure regulatory compliance.
