Job summary

Intermountain Health’s St Vincent Regional Hospital in Billings, MT seeks an RN Hospital Care Manager I to deliver comprehensive care management. You will screen, assess, plan, and coordinate care with patients, families, providers, and payers across inpatient and outpatient settings.

You will guide self-management through motivational interviewing and education, work with multidisciplinary teams, and lead care-transition improvements while ensuring regulatory compliance.

Qualifications

• Current RN license in state of practice.
• BSN within four years of hire/promotion.
• Inpatient hospital nursing experience.
• Understanding of disease management and barriers to care.

Responsibilities

• Identify patients using screening criteria and conduct comprehensive evaluations.
• Develop, adjust, and coordinate care plans addressing medical and social needs.
• Guide self-management using motivational interviewing and coaching.
• Educate and advocate for patient rights and self-care.

• Lead initiatives to enhance care transitions and policy adherence.
