Independent Living Systems (ILS) is seeking an RN Care Manager to coordinate care for Medicaid/Medicare Special Needs programs in Deerfield Beach, FL. The role emphasizes assessing needs, developing person-centered plans, and ensuring seamless service access.

The RN will work with healthcare providers, social workers, families, and community resources to align care with clinical guidelines and program requirements while promoting independence and quality of life for members.

Competenze

• Florida-licensed RN with at least 2 years' experience with disabled and elderly populations
• At least 2 years in care management or care coordination
• Proficiency with EHR and basic computer applications
• CCM certification preferred

• Assess member needs and develop person-centered care plans
• Coordinate services across the continuum of care and facilitate transitions
