Independent Living Systems (ILS) is seeking an RN Care Manager in Pompano Beach, FL to coordinate comprehensive care for Medicaid and Medicare Special Needs Program members. The role involves assessing needs, developing person-centered plans, and ensuring seamless service delivery with multidisciplinary teams.

Ideal candidates hold a Florida RN license with at least two years in care management or similar roles, plus EHR proficiency and familiarity with community resources.

Competenze

• Florida-licensed RN with at least 2 years of experience with disabled and elderly populations.
• 2 years of experience in care management or related healthcare role.
• Proficiency in EHR and basic computer applications.

Mansioni

• Assess member needs and develop person-centered care plans.
• Monitor progress and adjust plans as needed to changing needs.
• Coordinate access to services across the continuum of care.
• Educate and support on treatment adherence, disease management and self-advocacy.
