Job summary

Washington Regional is seeking a Care Manager (CM) to support patients across the care continuum, performing nurse-led Medicare AWVs, case management, and APCM enrollment. The CM will work with providers and clinical operations within Epic to document, track, and coordinate patient care activities.

The ideal candidate holds an Arkansas RN license with at least two years of primary care or care coordination experience, and has familiarity with Medicare AWV, CCM, and APCM program requirements.

Qualifications

• Current, unencumbered RN license in Arkansas.
• Minimum 2 years of clinical experience in primary care, case management, or care coordination.
• Strong understanding of Medicare AWV, CCM, and APCM program requirements.
• Proficiency with EHR systems (Epic preferred).

Responsibilities

• Perform nurse-led Medicare Annual Wellness Visits (AWVs) and document for CMS billing.
• Conduct case management and complex care management for patients with chronic needs.
• Identify eligible patients for APCM and enroll with patient/caregiver education.
• Coordinate referrals and collaborate with the care team to close care gaps and improve quality outcomes.
