Job summary

Community Clinic is seeking an RN Care Manager for our Primary & Specialty Care Clinic on MLK Blvd in Fayetteville, AR. The RN Care Manager will engage with high-risk patients, guiding self-management and care planning and managing transitions. In-person, Monday–Friday, 8:00 AM–5:00 PM.

The role emphasizes coordinating social services, compliance with regulatory standards, and collaboration with clinical staff to achieve goals and improved patient outcomes.

Qualifications

• Arkansas RN license required.
• 2+ years of case/care management experience.
• High school diploma or equivalent.
• Experience in ambulatory/family practice preferred.
• Knowledge of HIPAA/HITECH and regulatory standards.

Responsibilities

• Provides case management/utilization review and planning to assure progression through the continuum of care.
• Coordinates integration of social service function into patient care.
• Follows population health management and chronic disease protocols.
• Ensures compliance with quality patient care and regulatory standards.
• Collaborates with clinical staff to develop and execute the plan of care.
• Enhances collaboration to help patients and families make informed decisions.
• Ensures appropriate and timely patient tests and lab results.
• Promotes efficient utilization of clinical resources.
