Job summary

ArchWell Health is seeking a Nurse Care Manager to coordinate and evaluate management of patients with acute and chronic conditions across the care continuum. The role involves developing care plans with the care team and executing them to meet performance standards, regulatory compliance, and quality patient care.

The Nurse Care Manager will collaborate with clinical staff, coordinate transitions of care, and guide patients from our centers into acute and post-acute facilities and home

Qualifications

• Bachelor’s degree in nursing (BSN) or RN with related clinical field preferred.
• Active RN license in state of employment required.
• Willingness to obtain RN license in other states with ArchWell Health centers.
• CCMC/CMCN certification preferred or willingness to obtain within specified time.
• Minimum of 2 years’ clinical work experience.
• Minimum of 1 year in utilization review, home health, discharge planning.
• Minimum of 1 year of case management in acute or ambulatory settings.
• ICD-10, CPT, HCPCS knowledge preferred.
• Managed care experience with knowledge of HEDIS and CMS guidelines preferred.
• Spanish or other language skills helpful in communities served.

Responsibilities

• Establish a trusting relationship with patients, families, and caregivers.
• Collaborate with clinical staff to achieve patient goals.
• Reduce network costs by guiding referrals to preferred groups.
• Assess comprehensive needs and develop individualized care plans.
• Perform case management per CMSA standards.
• Provide patient education and care coordination to achieve health goals.
• Use Motivational Interviewing to engage patients.
• Communicate with hospital case managers, PTs, social workers, patients and families.
• Manage transitions of care and discharge planning.
