Qualifications

• RN license required by state.

• Strong communication and interpersonal abilities.

Responsibilities

• Facilitates communications among patient/family and care teams to resolve barriers.
• Collaborates with multidisciplinary teams to ensure quality, cost-effective care.
• Uses evidence-based approaches to engage patients/families in self-management.
• Manages CM caseload per department expectations with timely documentation.
• Coordinates referrals to internal and external resources.
• Ensures age-appropriate care and adherence to policies.
