Job summary

Qualifications

• BSN must be obtained within 7 years for those starting in this RN role.
• 2 years of relevant experience in clinical nursing and/or case management/managed care or related field required.
• 5 years of relevant experience preferred.

Responsibilities

• Assess needs via referrals and plan care across the continuum.
• Develops plan of care and makes recommendations to PCPs and specialists.
• Assess educational needs and implement teaching strategies or referrals.
• Coordinate services with providers and payers to improve access and outcomes.
• Measure patient care outcomes and analyze data trends for groups of patients.
• Document patient data, plan, interventions and outcomes per guidelines.
