Qualifications

Responsibilities

• Conduct telephonic, home visits, or in-person contacts as required.
• Monitor and revise care plans based on progress and outcomes.
• Maintain caseload and coordinate services across LTSS and behavioral health.
• Facilitate ICT meetings and support team collaboration.
