Job summary

Providence St. Joseph Hospital Eureka, CA is hiring a full-time RN Care Manager to oversee case management for complex patients across the continuum.

The role requires 8-hour shifts and strong communication with the care team to ensure efficient resource use and optimal patient outcomes. Qualified candidates will hold an ADN or nursing diploma and CA RN licensure, plus 2 years of acute care experience and 1 year in care management or a related orientation program.

Qualifications

• RN with CA license; ADN or diploma required.
• 2 years of acute care experience in Medical Surgical or Inpatient setting.
• 1 year in care management or utilization review or complete Case Management Orientation after hire.

Responsibilities

• Plan, coordinate, negotiate and manage care for complex patients and families across the continuum.
• Communicate daily with medical staff and hospital personnel to ensure appropriate resource use.
• Monitor medical records for variances to clinical/financial standards and intervene to optimize outcomes.
• Incorporate risk management and infection control knowledge to support the care team.
