Job summary

Humana/iCare seeks an RN Care Manager to join the Family Care Partnership program in Racine, WI. The role focuses on case management and authorizing services for adults in the community, with travel across several counties to conduct visits and assessments.

This full-time position offers a comprehensive benefits package and a schedule of Monday–Friday, 8:30 am–5:00 pm CST, with up to 50% travel. The RN will engage with members and families to promote health and safety.

Qualifications

• BSN preferred for the RN Care Manager role.
• 2+ years of related health care experience are required.
• Current RN License in Wisconsin is required.

Responsibilities

• Conduct home and community visits to complete nursing assessments.
• Complete the nursing portion of the comprehensive assessment.
• Monitor wellness and health promotion activities based on member assessments.
• Re-evaluate member health and wellness at appropriate intervals.
• Perform routine health monitoring activities based on member needs.
• Build constructive working relationships with members and their families.
• Provide assessment of psychiatric, behavioral, or medical problems in crisis situations.
• Evaluate members' ability to manage medication and medical care.
• Provide or arrange services that meet members' identified needs.
• Assist with program, policy, and procedure development.

• Pay: $71,100 - $97,800/year
• Location: Racine, WI
• Department/Unit: Family Care Partnership
• Schedule: Full-time, Monday - Friday, 8:30 am - 5:00 pm CST
• Travel: Up to 50% throughout Racine, Kenosha, Waukesha, Washington, Ozaukee, Walworth, and Sheboygan counties

What You'll Do
