Job Summary / Purpose

The RN Client Success Manager is responsible for ensuring that TheKey delivers consistent, high-quality care to our clients. This includes maintaining accurate and current care plans through regular reassessments and reports, monitoring care delivery, analyzing when additional or different care is needed, and building strong relationships with clients and their support systems. The role involves addressing physical, emotional, and social well-being, creating life-enriching opportunities, and promoting independence. Additionally, the RN Client Success Manager plays a key role in ensuring smooth operations, client satisfaction, long-term case retention, and collaboration with team members and the broader community.

Essential Duties and Responsibilities
  1. Manage daily client relationships and ensure delivery of quality care.
  2. Adopt a holistic approach to client care, focusing on quality of life, and recommend improvements to enhance client care and experience.
  3. Conduct in-person initial assessments and reassessments, including home safety evaluations, routines, preferences, and needs, translating findings into tailored care plans.
  4. Utilize interpersonal skills to build rapport with clients and support systems.
  5. Ensure the care team is updated with relevant information and documentation is thorough and current.
  6. Communicate proactively with internal and external stakeholders through meetings and updates.
  7. Consult with clients to understand their needs and preferences, and evaluate solutions.
  8. Serve as a subject matter expert for quality control and assurance matters.
  9. Ensure compliance with all state regulations, visits, and documentation requirements.
  10. Verify that caregivers meet state requirements, are properly trained, and receive RN supervision as needed.
  11. Conduct thorough initial assessments for new clients or review existing assessments.
  12. Follow the Start of Care process to ensure a positive onboarding experience.
  13. Perform reassessments and quality assurance visits, including safety and routine evaluations.
  14. Proactively reassess client needs, referring to additional services or adjusting care as necessary.
  15. Coordinate with the interdisciplinary team to ensure quality, appropriate caregiver matching, accurate billing, and benefits utilization.
  16. Manage escalations and complaints, resolving issues with judgment and discretion.
  17. Review and adjust client rates based on evolving needs and satisfaction.
  18. Ensure adherence to all regulations and company policies.
  19. Recommend or implement policies to address specific client needs.
  20. Identify cross-referral opportunities and avenues to grow business.
  21. Provide on-call and after-hours support as needed.
  22. Perform additional duties as assigned.
Required Skills, Education, and Certifications
  • Current RN license in the state of employment; continued employment contingent upon maintaining active license in good standing; BSN preferred.
  • At least 5 years of experience in healthcare, elder care, social work, or related fields preferred.
  • Excellent customer service and conflict resolution skills.
  • Proficiency in computer systems and accurate documentation skills.
  • Valid driver’s license and proof of insurance.
Physical Requirements
  • Ability to travel locally up to 75% of the time.
  • Ability to lift and carry up to 15-20 pounds.
  • Ability to sit, stand, and walk for extended periods.
  • Ability to operate standard office equipment.
