Job description

Title: Care Manager (RN)

Duration: 03 Months (Contract to Hire)

Location: Remote FL - Candidate must reside within the Broward region. (Member Facing 75-80%)

Job description:

Develops, assesses, and facilitates complex care management activities for primarily physical needs members to provide high quality, cost-effective healthcare outcomes including personalized care plans and education for members and their families.

Education/Experience:

Requires a Degree from an Accredited School of Nursing or a Bachelor's degree in Nursing and 2 - 4 years of related experience.

License/Certification:

RN - Registered Nurse - State Licensure and/or Compact State Licensure required.

Responsibilities include:
  • Evaluate the needs of the member, barriers to accessing appropriate care, social determinants of health, focusing on member priorities, and recommend or facilitate plans for the best outcomes.
  • Develop ongoing care and service plans, collaborating with providers to identify resources to address unmet needs.
  • Identify problems/barriers to care and provide appropriate interventions.
  • Coordinate between members, families, caregivers, and care teams to ensure person-centered care.
  • Provide ongoing follow-up and monitoring, revising care plans as needed.
  • Support members with local resources for various services based on assessments.
  • Facilitate care management and collaborate with providers for timely access to care.
  • Perform outreach via telephonic, digital, or home visits to assess needs.
  • Maintain all documentation to ensure compliance with regulations.
  • Educate members and families on health conditions, care gaps, and healthcare options.
  • Provide feedback to leadership for quality improvement.
  • Other duties as assigned to meet business needs.
Additional Education/Certification Details:
  • Required: Degree from an accredited nursing school or a bachelor's in nursing with 2-4 years of experience.
Experience Requirements:
  • Minimum 2 years pediatric experience.
  • Case management experience preferred.
  • Willingness to perform home visits.
