Role Overview

The Care Manager enhances patient care through outreach, care management, preventive services, and self-management support. They collaborate with primary care teams to meet quality and care goals.

Key Responsibilities
  • Serve as a subject matter expert in quality and value-based care programs.
  • Build relationships with providers and staff to improve follow-up care and health outcomes.
  • Develop tools, education, and workflows to meet CMS, ACO, and payor requirements.
  • Collaborate with interdisciplinary teams to meet care, quality, and financial goals.
  • Conduct meetings and provide education on program goals and results.
Patient Care and Coordination
  • Monitor patient compliance with screenings and behavioral health management.
  • Use portals and reports to evaluate progress and plan workload.
  • Communicate with patients via phone and virtual tools, documenting appropriately.
  • Maintain patient records and assist with documentation for payor projects.
  • Support transition of care activities post-hospital or ED visits.
Care Management and Population Health
  • Triages patients for appropriate care management.
  • Develop care plans with patients and caregivers.
  • Enroll patients in chronic or longitudinal care programs.
  • Coordinate specialty referrals and follow-up care.
  • Assist with practice and provider empanelment and scheduling.
Qualifications
  • Graduate of an accredited nursing program.
  • Knowledge of physician practice operations; 1+ year experience preferred.
  • Current RN licensure in the state of residence/practice.
