Role Overview
The Care Manager enhances patient care through outreach, care management, preventive services, and self-management support. They collaborate with primary care teams to meet quality and care goals.
Key Responsibilities
- Serve as a subject matter expert in quality and value-based care programs.
- Build relationships with providers and staff to improve follow-up care and health outcomes.
- Develop tools, education, and workflows to meet CMS, ACO, and payor requirements.
- Collaborate with interdisciplinary teams to meet care, quality, and financial goals.
- Conduct meetings and provide education on program goals and results.
Patient Care and Coordination
- Monitor patient compliance with screenings and behavioral health management.
- Use portals and reports to evaluate progress and plan workload.
- Communicate with patients via phone and virtual tools, documenting appropriately.
- Maintain patient records and assist with documentation for payor projects.
- Support transition of care activities post-hospital or ED visits.
Care Management and Population Health
- Triages patients for appropriate care management.
- Develop care plans with patients and caregivers.
- Enroll patients in chronic or longitudinal care programs.
- Coordinate specialty referrals and follow-up care.
- Assist with practice and provider empanelment and scheduling.
Qualifications
- Graduate of an accredited nursing program.
- Knowledge of physician practice operations; 1+ year experience preferred.
- Current RN licensure in the state of residence/practice.
