The Nurse Case Manager (NCM) is responsible for managing a caseload of patients across the healthcare continuum to ensure optimal quality outcomes, patient satisfaction, and appropriate resource utilization. The NCM leads an organized, patient-centered interdisciplinary team that coordinates healthcare services by monitoring, evaluating, and modifying treatment plans to achieve the best patient outcomes and resource efficiency.

Important role details:

• This position is remote.
• Work status and shifts available: Per Diem / Day shift.

Required Qualifications:

• Graduate of an accredited nursing school.
• Montana RN license upon hire.
• At least 3 years of clinical nursing experience.
• At least 1 year of hospital case management experience.

Preferred Qualifications:

• Current Case Manager Certification upon hire.
