Join Vancouver Clinic as an RN Care Manager in the Population Health team. This remote role in Washington requires coordinating patient care, aiming to enhance the quality of service provided. A competitive compensation package with comprehensive benefits is included for full-time employees.
We are looking for an RN Care Manager / Disease Manager to join our Population Health team!
Location : Remote - based in Washington or Oregon
Schedule: Monday-Friday, 8am-5pm. Option for five 8/hr days OR four 10/hr days
Compensation: Hourly compensation minimum is $44.97, dependent on qualifications and professional experience. Base Medical, Dental, and Rx Insurance premium is 100% paid for by Vancouver Clinic for full-time employees.
Position Summary: This person will work within the Population Health division to coordinate care for patients, as well as work to improve the quality of life, care, and service experienced by patients and their family. They will also provide continuity of care facilitating a seamless, coordinated process that supports the patients need for education, care coordination, and psychosocial support.
Position Duties:
Screen, assess, and prioritize patients and referrals.
Identify issues, trends, and barriers that delay the patient's timely transition to the next level of care, and accurately communicate this information to others.
Influence actions and opinions in cases where a physician needs assistance to expeditiously transition a patient to the most appropriate level of care.
Utilize the nursing process approach to prospectively and concurrently formulate and execute plans for health care services.
Identify where support structures exist, identify appropriate referrals and resources, take action, and make commitments to solve individual patient care problems.
Develop creative patient care alternatives to ensure the delivery of high quality, cost effective care using analytical problem solving as well as intuition to stimulate new waves of thinking.
Discuss complex or difficult individual cases with supervisor and physicians.
Identify and report potential risk cases or situations by presenting clear information that builds credibility.
Requirements:
Graduate of an accredited School of Nursing (Registered Nurse).
Current unrestricted Washington State RN license.
Current unrestricted Oregon RN license within 60 days of hire or transfer required.
American Heart Association Health Care Provider BLS CPR certification.
Minimum of two years clinical care experience as an RN required
Experience in care management or home health strongly preferred.
First-line problem solving skills with the ability to listen, understand, and solve or escalate employee and provider issues before they become more serious.
Pay Range:

Benefits-eligible employees accrue PTO and Personal Time based on hours worked and State worked, totaling 120 hours in the first year for full time staff and 200 hours in the first year for full time supervisors and above, increasing in subsequent years. PTO and Personal Time accruals are pro-rated by FTE/hours worked. Non-benefits eligible employees will accrue Personal Time based on hours worked and State worked.
